Program Management / Managing Change - Identifies, develops and implements VISN-wide targeted programs to achieve performance measures and clinical guideline, quality, or regulatory requirements.- Provides oversight and coordinates practice groups of clinical pharmacy specialists (CPS) in their respective areas from the VISN-17 facilities;- Facilitates workgroup members' participation, effectively promotes collaborative identification of actionable items, ensures appropriate reporting of progress towards selected goals, and provides sufficient administrative support to organize and conduct meetings.- Utilizes various databases and informatics tools to analyze prescribing practices and identify providers or groups for targeted education;- Actively champions the promotion of VA National formulary and evidence-based, safe, and cost-effective drug therapy during visits.- Participates in review and analysis of reports providing comparative data on clinician panels at VISN-17 facilities;- Performs data analysis to measure performance and to trend practice changes;- Consults with VISN and PBM staff to help initiate, develop, validate and execute projects;- Executes small-scale educational interventions for time-sensitive campaigns to change prescribing patterns including, but are not limited to, PBM updates, polypharmacy containment, formulary process and controls, and approved drug conversions- Works closely with VISN oversight committees (e.g. VISN P&T, VISN P&T Subcommittees, local or National committees, facility leadership, etc.) to target initiatives and strategies unique to the medical facilities.- Provides consistent follow-up to individual questions/requests, is available for lectures/in-service presentations related to hematology/oncology topics as requested by facilities and advise implementation strategies for planned activities as needed.
Safe and Cost-Effective Medication Use- Serves as the Clinical Pharmacy Subject Matter Expert (SME) for hematology and oncology medications, including new medications approved by the FDA;- Develops strategies for optimal use of these medications with consideration to patient safety, quality of care, and financial impact;- Applies marketing techniques in promotion of clinical practice guidelines and other evidence-based prescribing initiatives and practices;- Provides subject matter expertise on hematology/oncology medication compounding to the VISN Compounded Sterile Products committee to ensure VISN-wide compliance and adherence to USP 797, USP 800, and CETA-CAG specifications as well as Joint Commission and FDA requirements;- Engages in educational/academic detailing activities on topics identified by national PBM and identified within the network;- Provides feedback on New Drug Monographs and Criteria for Use being developed by PBM at the National level for drugs within the assigned area of expertise.- Reviews prior authorization drug requests and population health reports for VISN 17 facilities and assess for clinical appropriateness by utilizing clinical knowledge, national and VISN treatment guidelines, drug-use criteria, and relevant literature and resources.- Communicates to appropriate VISN or facility leadership recommendations for change in prescribing stemming from review of literature or prior authorization requests.
Customer Service- Demonstrates sensitivity to and an understanding of a wide variety of staff, patients, caregivers and other customers- Consistently responds to VISN 17 PBM program inquiries, medication or other relevant requests in a timely, accurate, friendly, and professional manner across the spectrum of VISN 17 PBM internal and external customers.- Routinely identifies the customer's situation properly, and performs the tasks required to respond to the customer and resolve the issue(s).- Routinely conducts necessary follow-up to ensure a satisfactory resolution.- Listens to customer feedback, positive and/or negative, and acts to resolve matters/situations within his/her control- Questions or issues not within the employee's control are referred in a timely manner to appropriate staff able to assist with the situation.- Strives to exceed customer expectations.

Qualifications

Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education.
- Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
- Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
- English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
- May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Preferred Experience:
- ASHP-Accredited PGY2 Residency in Oncology
- Board of Pharmacy Specialties Certification (BCOP)
- 5+ years of experience as an outpatient Clinical Pharmacist Practitioner (CPS with a scope of practice) in Oncology
- 3+ years of experience performing Comprehensive Medication Management via video/virtual modality of care (Virtual Care Manager, VA Video Connect, Clinical Video Telehealth)
Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Assignment: A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.
Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
